11(a.) / (b.) Secondary Multi-Needs Program
 

10. ELEMENTARY MULTI-NEEDS PROGRAM

Location: West Elementary School, 1322 Central St., Stoughton, MA   02072

Student Age Range: 7-11 Years

General Developmental/Instructional Level:

  • Skills-development in range 6 months to 4 years
  • Severe or profound developmental disabilities; multiply-handicapped

Specific Needs, Disabilities, Treatments & Programming Methods:

  • Mental retardation
  • Cerebral palsy
  • Visual impairment
  • Seizure disorder
  • Hearing impairment
  • Other central nervous system disorders
  • Intensive staff:student ratio; nursing, physical therapy, occupational therapy and speech therapy personnel to meet developmental, communication, functional, and health goals

Entrance Criteria:

  • First or continuation public school placement for young children with severe or profound developmental disabilities and functional  handicaps
  • Needs in the domains of physical disability and medical support that can be met in a public school environment with intensive staff:student programming ratio

Exit Criteria:

  • Transition to secondary school program by age 12
  • Medical or social context requiring more restrictive environment

11(a.) / (b.) Secondary Multi-Needs Program

Location:  Holbrook Junior/ Senior High School, 245 So. Franklin Street,  Holbrook, MA 02343

Student Age Range:  (A) 12-16 Years ,  (B) 16-22 Years

General Developmental/Instructional Level:

  • Skills in the ranges of pre-academic to functional academic
  • Intensive developmental disabilities
  • Secondary behavioral handicaps

Specific Needs, Disabilities, Treatments/Programming Methods:

  • Mental retardation
  • Autism
  • Cerebral palsy
  • Visual impairment
  • Seizure disorder
  • Hearing impairment
  • Other central nervous system disorders
  • Related disturbances of behavior and social relationships
  • Programming staff: student ratio of 1:2 to meet developmental, behavioral, and communication goals; for profound disabilities, intensive services in allied health fields to meet developmental, functional, and health goals

Entrance Criteria:

  • Continuation public school placement for pre-adolescent and adolescent youth with severe and profound developmental disabilities and related functional handicaps
  • Needs in the domains of behavior, communication, self-care, activities of daily living and pre-vocational training, which can be met with 1:2 programming ratio; or, for profound disabilities, physical and medical needs that can be met with intensive in-school services

Exit Criteria:

  • Developmental functioning beyond the 6-year level, or workshop vocational capability at 25% of non-handicapped rate
  • Ability to acquire skills in lower-structure environment (e.g., elimination of interfering behavior; alleviation of communication disorder)
  • Transition to adult program at age 22
